Patellar Instability

Pathophysiology
Anthony A. Schepsis, MD

Next

Patellar instability is defined as abnormal, clinically symptomatic, lateral or, in rare cases, medial translation of the patella out of the trochlear groove.

Instability includes dislocations (figure right, click image to enlarge) as well as subluxations, both of which can be either traumatic or atraumatic.

Etiology
Etiology plays an important role in determining whether surgery is indicated, and if so, which operation is best.

For example, a congruent joint with traumatic onset will most likely need attention to the medial patellofemoral ligament (figure right, click image to enlarge).
