Assessment and Evaluation of Glenoid Bone Loss

The preoperative assessment of anterior glenoid bone loss is a critical step in surgical planning for patients with recurrent anterior glenohumeral instability. The structural integrity of the glenoid is one of the most important factors that affects the success of surgical repair. The currently accepted preferred method for structural assessment of the glenoid is three-dimensional CT reconstruction with the humeral head digitally subtracted, yielding an en face sagittal oblique view of the glenoid. This view allows for evaluation of glenoid morphology and quantitative assessment of glenoid bone loss. This video describes the practical application of the National Institutes of Health ImageJ software, which quantifies the amount of glenoid bone loss as a percentage of total surface area or diameter. In the diameter-based method, the percentage of bone loss = (defect width/diameter of the inferior glenoid circle) x 100%. In the surface area method, the percentage of bone loss = (defect surface area/surface area of the inferior glenoid circle) x 100%.
