Chapter 36 Video 6: Peritrochanteric Access and Gluteus Medius Repair

January 05, 2018

Contributors: J W Thomas Byrd, MD

This video demonstrates arthroscopic peritrochanteric access and gluteus medius repair. Two IT band sparing anterior portals are used, along with a posterior viewing portal and a distal working portal. The tear is longitudinal, so two longitudinally oriented triply loaded anchors are used. The sutures are passed through the posterior and anterior leaflets of the tear. The proximal anchor and then the distal anchor are implanted. The sutures are passed before being tied down. A gluteus medius repair with double row fixation is also demonstrated. Two or three transversely oriented doubly loaded anchors are used. All sutures are passed and tied down, one suture limb from each knot is left to incorporate into the distal fixation. A knotless fixation device is used for the distal row.
