Irreparable, chronic massive rotator cuff tears are a challenge to manage surgically. Many treatment options are available for the management of chronic massive rotator cuff tears. Superior capsule reconstruction is a treatment option that may restore and rebalance the force couples necessary for dynamic shoulder function. This video discusses an arthroscopic technique that involves the use of a hybrid graft consisting of autogenous tensor fascia lata and an allograft for superior capsule reconstruction in patients with a chronic irreparable rotator cuff tear. This technique provides excellent biologic potential of the patient's own autograft and strong mechanical properties of a volumetric allograft. The technique does not compromise future treatment for patients with an irreparable rotator cuff tear.
