Bony and Soft-Tissue Surgery for Refractory Patellofemoral Instability

This video discusses the case presentation of a 16-year-old female athlete with a 3-year history of recurrent patellar dislocation. The patient has several risk factors for recurrent patellar dislocation, including a lateralized force vector (elevated tibial tuberosity-trochlear groove distance), patella alta (elevated Caton-Deschamps ratio), high-grade trochlear dysplasia, and patellar tilt. The surgical techniques described in this video include tibial tubercle osteotomy (anteromedialization and distalization), entrance trochleoplasty, medial patellofemoral ligament reconstruction with the use of allograft, and lateral retinacular lengthening.
