Posterior glenohumeral instability has become increasingly recognized in the young, athletic patient population. In the past decade, considerable basic and clinical research has improved our understanding of the pathoanatomy of, biomechanics of, and appropriate treatment strategies for posterior shoulder instability. With improvements in arthroscopic instrumentation and implants, management of posterior shoulder instability via arthroscopic techniques has improved. This video describes the indications, workup, techniques, and outcomes for patients undergoing arthroscopic posterior shoulder stabilization for management of recurrent posterior shoulder instability. Surgical pearls, including patient positioning, surgical approaches, and portal placement, are reviewed in detail.
